Any person (resident, employee, ombudsman, relative, friend, other) may file a complaint regarding a facility staff person, a regulated health care facility, a clinical lab or other Division of Quality Assurance issue.

For complaints concerning health care provider types listed below, contact the Division of Quality Assurance toll free complaint line at 1-800-642-6552 or you may file a complaint online using the Online Complaint Intake System:

  • Ambulatory Surgical Centers

  • Clinical Laboratories or Analysts

  • End Stage Renal Dialysis

  • Home Health Agencies

  • Hospices

  • Hospitals

  • Outpatient Rehabilitation Agencies

  • Rural Health Clinics
