Substance Abuse Prevention Services Information System (SAP-SIS)The Substance Abuse Prevention Services Information System (SAP-SIS) is a web-based data collection tool developed by the State of Wisconsin, Department
of Health Services, and the Division of Mental Health and Substance Abuse Services (DMHSAS). Collection of this data is required to meet Federal SAPT Block Grant and National Outcome Measure reporting requirements. Data collected through SAP-SIS will be used by the State of Wisconsin to fulfill annual SAPT Block Grant reporting requirements. Agencies will use SAP-SIS to report on required data from their prevention efforts throughout the contract year. This system will take the place of previous paper and pencil prevention reports submitted by the primary contract agencies. To use SAP-SIS, primary contract agencies must:
After completing these steps and receiving the approval from the Division, primary contract agencies and subcontract agency users will be able to log on to SAP-SIS to enter required data for provided prevention services.
